What this website knows about you, in plain language.
Short version: almost nothing, and only what you choose to send us.
The contact form
When you submit the contact form, the details you type -- your name, email address, phone number, preferred contact method, whatever you selected under "what are you interested in," and your message -- are delivered to the 5 Star Impact team's inbox over an encrypted connection, through FormSubmit, our form-delivery processor. FormSubmit's own privacy policy governs the brief time your submission passes through their system. 5 Star Impact keeps what lands in that inbox only as long as needed to respond to you. We use your information to respond to you, and for nothing else -- we do not sell it, share it for marketing, or add you to a mailing list.
Please do not send Social Security numbers, financial account details, or detailed medical information through this form -- call us instead for anything like that. If you are asking about a youth program on behalf of a child, please have a parent or guardian submit the inquiry. If your question involves Adult Day Program care details, a phone call keeps that information more private than an email inquiry does.
Cookies and tracking
This website sets no cookies of its own and runs no analytics or advertising trackers. There is nothing to opt out of on our end, which is why you did not see a cookie banner.
The map
The contact page includes an embedded Google Map showing our location. That map loads automatically when you visit the page -- it is not gated behind a click -- and, like any Google service, it may set or read Google's own cookies in your browser. If you'd rather not load it, our street address is printed in plain text right beside it and works just as well for directions.
